Select “Transactions” and then “Business Entity Name
Request”
**Note:
– You do not need to change your company’s name.
– You will be required to transact all CA insurance business under a Doing
– Business As (DBA) name that meets the CA name approval criteria

– Once you submit the request, you should receive a
notice from CA DOI within 2-3 business days.
– In the email, you will find your BE Name Approval Confirmation Number.

It will take about 2-3 weeks for DOI to process your business entity application.

Once approved, you will receive an email from the DOI saying that your license is ready to print out online.
